Skills of Central PA is hiring a Licensed Practical Nurse (LPN) in Altoona, PA!

We are seeking a compassionate, dependable, and motivated LPN to support individuals with intellectual and developmental disabilities in our residential homes throughout Blair County.
This position offers a primarily Monday-Friday, 8:00 AM-4:00 PM schedule, with flexibility needed outside those hours to meet the needs of the individuals and staff we support.
What You'll Do
As an LPN at Skills, you'll play an important role in coordinating healthcare, promoting wellness, and providing guidance to the Direct Support Professionals who care for individuals each day.
Your responsibilities will include:
- Assessing and monitoring individuals' health, vital signs, and overall well-being.
- Coordinating medical appointments, treatments, and follow-up care.
- Administering medications and treatments as prescribed and within your scope of practice.
- Identifying health concerns and supporting preventive care.
- Training, educating, and providing ongoing guidance to Direct Support Professionals.
- Maintaining accurate medical records and nursing documentation.
- Communicating with healthcare providers, families, and team members to ensure quality care.
- Advocating for the health, dignity, rights, and independence of the individuals we support.
- Assisting with direct care and residential staffing coverage when needed.
What We're Looking For
- Current Pennsylvania Licensed Practical Nurse (LPN) license.
- Strong communication, organization, and problem-solving skills.
- A compassionate, patient, and person-centered approach to care.
- Ability to work independently while collaborating with a team.
- Flexibility and willingness to support staff and individuals when needs arise.
- Strong follow-through and attention to detail.
- Administrative and healthcare coordination experience preferred.
Schedule & Location
This position is based in Altoona, PA, with travel to residential homes throughout Blair County.
The primary schedule is Monday-Friday, 8:00 AM-4:00 PM. Some flexibility outside regular business hours is required for staff training, medical questions, direct care assistance, and occasional residential shift coverage.
