Nurse Practitioner - In-Home Health Assessments in Oregon - PRN or Full-time

Nurse Practitioner Job Details
- Location: Lane, Jackson, Marion, Multnomah, Polk, or Washington counties
- Work Setting: Field-based, in-home visits and home health assessments
- Employment Type: Local opportunity
- Schedule: Completely flexible
- Shift: 8-hour daytime shift scheduled within operating hours of 8:00 a.m. to 8:00 p.m., seven days per week
- Duration: Approximately 52 weeks
- Anticipated Start Date: November 2, 2026
- Visit Volume: Approximately 6-7 visits during an 8- or 10-hour day, depending on patient needs and location
- Role Focus: Preventive, non-prescribing health assessments, patient education, care-gap closure, and coordination with primary care providers
Nurse Practitioner Job Requirements
- At least 1 year of clinical experience; family practice, adult, geriatric, or home health experience is preferred
- Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
- Active, unrestricted Nurse Practitioner license in Oregon or the ability to obtain Oregon licensure before the start date
- Current national provider credentials and an unrestricted practice history without active disciplinary restrictions
- Active, unrestricted driver's license and reliable transportation for in-home patient visits
- Proficiency with electronic medical records (EMR) and mobile technology
- Ability to work independently in varied home environments and navigate stairs
- Ability to sit, stand, kneel, and transport equipment weighing up to 30 pounds as needed
- Strong communication skills and a patient-centered approach when working with older adults and Medicare populations
In-Home Nurse Practitioner Responsibilities
- Conduct comprehensive in-home health assessments, including medical history review, medication reconciliation, vital signs, and physical examinations
- Complete evidence-based health screenings and appropriate point-of-care testing
- Identify and document diagnoses to support care management and treatment planning
- Communicate assessment findings and care gaps to members' primary care providers
- Recognize urgent or emergent conditions and take appropriate action or facilitate timely referrals
- Educate patients and caregivers about disease processes, medications, preventive care, and treatment adherence
- Address social determinants of health and connect patients with appropriate community and support services
- Provide care directly in patients' homes while maintaining professionalism, safety, privacy, and accurate clinical documentation
- Participate in training and orientation for assessment protocols, documentation standards, and point-of-care procedures
- Coordinate availability and county coverage to support consistent scheduling and patient access
