Loudoun Home Care is looking for a compassionate, dependable, and attentive Licensed Practical Nurse (LPN) to join our growing home care team.

As an LPN, you'll provide personalized nursing care in the comfort of a client's home while working closely with the supervising RN, client, family, and care team.


PAID TRAINING & ORIENTATION PROVIDED

You will receive training and hands-on orientation before independently starting your shift.

We want you to feel confident and prepared before providing care independently. Training and orientation will be provided based on the client's specific care needs, including specialized care such as tracheostomy and G-tube care.

What You'll Do


As an LPN, you will provide nursing care according to the client's individualized plan of care and under the direction of the supervising RN.

Responsibilities may include:

  • Provide nursing care according to the client's plan of care and RN direction.
  • Administer medications and perform approved treatments and procedures.
  • Provide specialized care for clients with tracheostomies and G-tubes.
  • Monitor the client's condition and promptly report changes or concerns to the supervising RN.
  • Complete accurate and timely clinical documentation.
  • Follow infection-control, safety, and agency procedures.
  • Educate and support clients and their families regarding care needs.
  • Communicate effectively with the supervising RN, family members, caregivers, and other members of the care team.
  • Protect client privacy and follow HIPAA requirements.
  • Participate in required training, continuing education, and quality-improvement activities.
  • Promote a safe, respectful, and comfortable environment for every client.


What We're Looking For


  • Active Virginia or Compact LPN license
  • At least 1 year of nursing experience preferred
  • Current CPR/BLS certification
  • Experience caring for clients with tracheostomy and G-tube preferred
  • Home care, private duty nursing, or community health experience is a plus
  • Ability to recognize and appropriately report changes in a client's condition
  • Strong communication, documentation, and organizational skills
  • Compassionate, dependable, attentive, and professional approach to client care
  • Ability to work independently while following the established plan of care
