Requirements:
  • Physicians must be board certified in their specialty, or board eligible and will become board certified within 2 years of becoming board eligible in Gastroenterology - REQUIRED (no exceptions)
  • Valid MI license - REQUIRED - at time of employment.
  • DEA - REQUIRED
  • BLS Certification and maintain recertification - REQUIRED
  • Most recent NPDB Self Query Report (within 30 days active) - REQUIRED no exemption, required at presentation
  • Board Certification (or Board Eligibility with plan to certify within 2 years)
  • BLS Certification (with maintenance of recertification)
  • DEA
  • BLS
  • MI License
  • Board Certified
  • Board Eligible
  • DEA
