What You ll Do - Provide outpatient geriatric medical evaluations and management of chronic, complex conditions
- Conduct preventive care and health screenings for senior patients
- Collaborate with a dedicated team to develop personalized treatment plans
- Manage medication protocols and coordinate care with other health professionals
- Document patient visits accurately using Epic EMR system
- Participate in after-hours call coverage approximately one week per year, including weekends
- Support patient education on health maintenance and disease prevention
What You Need - Active medical license, board certified in Internal Medicine or Family Medicine
- Minimum 1 year outpatient primary care or geriatrics experience
- Strong clinical skills with geriatrics and chronic disease management
- Compassionate communication and patient-focused approach
