What You'll Do:
- Provide comprehensive primary care in an office/clinic setting, including wellness exams, preventive care, and chronic disease management
- Conduct in-home patient visits (typically 2 3 days per week), delivering personalized, home-based care
- Evaluate, diagnose, and treat a broad range of acute and chronic conditions across a diverse patient population
- Coordinate care with on-site specialists (urology, cardiology, gastroenterology) and ancillary services (PT, mental health, pain management)
- Maintain accurate clinical documentation scribe support available for providers who want extra help with EMR
- Build deep, long-term patient relationships through ongoing follow-up and support
What You'll Bring:
- NP (AGNP or FNP), or PA with active or eligible NY licensure
- Excellent interpersonal skills and a strong commitment to patient-centered care
- Comfort with both in-office and in-home visit formats
- Russian language skills required for this location
