Requirements:

Board Certified in Neonatology (not accepting board eligible) - REQUIRED
MD or DO degree - REQUIRED
Virginia Medical License (or willingness to obtain) - REQUIRED
Leadership experience in NICU or clinical program development - REQUIRED
Neonatology experience - REQUIRED (experienced candidates only)
Does the provider require visa support (J1/H1B)? - REQUIRED to specify
Most recent NPDB Self Query Report (within 30 days active) - REQUIRED no exemption, required at presentation
DEA (must be obtained prior to start) - (note in "Additional Submission Details" field) - REQUIRED no exemptions
MD or DO degree
Eligible for hospital credentialing
Leadership experience in clinical quality or program development (implied requirement)
Board Certified
