Schedule: Full-Time Outpatient Monday Friday
Outpatient Primary Care Adult & Senior Population A well-established outpatient primary care practice in Reading, PA is seeking a Primary Care Physician (Internal Medicine or Family Medicine) to join a team-based, value-focused care environment serving adults and seniors.
This opportunity is ideal for physicians who want time with patients, continuity of care, and a predictable outpatient schedule, while practicing within a supportive, well-resourced clinical model.
Position Highlights Outpatient primary care only
Monday Friday schedule
No hospital rounding or inpatient responsibilities
Focus on adult and geriatric patients
Team-based care with strong clinical and administrative support
Emphasis on quality, outcomes, and preventive care
Key Responsibilities Provide comprehensive primary care for adult and senior patients
Manage chronic conditions and preventive health services
Build long-term patient relationships
Collaborate with interdisciplinary care teams
Practice within established clinical guidelines and quality measures
Participate in population health initiatives
Qualifications Board Certified or Board Eligible in Internal Medicine or Family Medicine
Eligible for or active Pennsylvania medical license
Comfortable caring for older adults with multiple chronic conditions
Interest in preventive and value-based care
