What You'll Do:
- Provide comprehensive primary care to a diverse patient population, from routine wellness visits to chronic disease management
- Build long-term relationships with patients and their families, delivering personalized, continuity-focused care
- Collaborate with a network of specialists
- Diagnose, treat, and manage a wide range of acute and chronic conditions
- Order and interpret diagnostic tests, and develop individualized treatment plans
- Coordinate referrals and communicate seamlessly with specialty partners to ensure continuity of care
- Maintain accurate, timely documentation within the practice's EMR system
What We're Looking For:
- MD or DO with active (or eligible) state medical license
- Board certified or board eligible in Family Medicine or Internal Medicine
- Strong communication and interpersonal skills with a genuine passion for patient care
- Comfortable working in a collaborative, multi-specialty environment
- Committed to providing sensitive, high-quality, individualized care
- New graduates and experienced physicians both encouraged to apply
