Qualifications:
- MD or DO from an accreditedmedical school
- Active Arizona medicallicense (or eligibility to obtain)
- Current DEA registration
- Board Certified or BoardEligible in Family Medicine or Internal Medicine
- Experience caring forgeriatric, post-acute care, SNF, LTC, or medically complex patient populationsis preferred
- Excellent clinical judgment,communication, and team collaboration skills
