Practice Highlights / Qualifications- Outpatient primary care setting
- Value-based, senior-focused care model
- Collaborative practice under a supervising physician
- Interdisciplinary team support and technology-enabled care model
- EHR: Elation
- Master of Science in Nursing from an accredited program required
- ANCC or AANP certification required
- Texas Nurse Practitioner license required or ability to obtain
- Family, Adult Gerontology, or Primary Care certification preferred
- Must meet employer credentialing standards
- Senior care, gerontology, post-acute, or value-based care experience preferred
- Spanish language skills preferred
