What You ll Do
- Provide outpatient geriatric care to adults aged 55 and older within a PACE program
- Conduct in-clinic and in-home visits, including travel to assisted living, SNFs, and private residences
- Collaborate with an 11-member interdisciplinary care team
- Manage mostly elderly patients with chronic and acute conditions
- Utilize EMR for documentation and patient management
- Participate in full patient assessments and coordinate community-based care
- Maintain adherence to all licensing and certification standards
What You Need
- Active medical license in Oregon
- Current DEA registration
- BLS certification
- Geriatric or internal medicine experience (minimum 2 weeks/month week commitment)
- Strong communication skills and team-oriented mindset
- Ability to work independently in a community care setting
