We are hiring an experienced Nurse Practitioner (NP) to provide high-quality medical care in Skilled Nursing Facilities (SNFs). This is a stable, full-time W2 position offering competitive compensation, strong clinical autonomy, and a predictable weekday schedule.
This role is ideal for an NP who enjoys post-acute care, values collaboration, and is comfortable managing patients independently in an SNF setting.

Key Responsibilities
- Perform comprehensive patient assessments in SNF settings
- Diagnose and manage acute and chronic medical conditions
- Conduct routine patient rounds
- Collaborate with physicians, nursing staff, and care teams
- Order and interpret labs, diagnostics, and treatments
- Document all patient care accurately in the EMR
Qualifications
- Active Nurse Practitioner license (state-required)
- Experience in Skilled Nursing Facilities, post-acute, or long-term care required
- Strong clinical judgment and decision-making skills
- Ability to work independently while collaborating with a care team
- Comfortable managing a patient panel in an on-site setting
