This full-time opportunity offers a Monday through Friday schedule (8:00 AM to 5:00 PM) serving patients throughout the Antelope Valley, San Fernando Valley, Conejo Valley, and Simi Valley. You'll provide acute, post-acute, and transitional care services in patients' homes, helping individuals successfully recover after hospitalization, manage chronic conditions, and avoid unnecessary readmissions. Supported by a collaborative interdisciplinary team, you'll have the autonomy, resources, and time needed to deliver personalized, relationship-centered care where it matters most.
