COMPENSATION: $300K+ Signing Bonus

We are a unique physician-led culture that provides coordinated, world-class primary care to the most vulnerable population, moderate to low-income seniors with complex chronic diseases. We are able to practice medicine the way it should be practiced. We build relationships with our patients the old fashion way, we have a reduced patient panel that allows us to see them with a higher frequency. Simply put, we give our patients the time and respect they deserve and require.

Role:

• Compensation: $300K+
• Bonus: up to 10% of Base Salary
• Partnership track
• 18-20 patients a day
• Hours: Monday Friday work schedule; Clinic Hours 7:30a 5:00p (last patient is scheduled at 4:15)
• 100% outpatient
• Dedicated Care and support team (have your own MA/Scheduler/Referral specialist)
• Patient Population: Seniors with 3-5 chronic diseases
• Value Based Care

Requirements:

• Residency Trained IM/FM
• Board Certified/Board Eligible in: FM, IM or Geriatric Medicine
• Bilingual: Spanish Speaking
