Responsibilities
• Care for a spectrum of primary care concerns on a given day, seeing a panel of families and individuals through both chronic illnesses and acute concerns.
• The provider is the anchor of an integrated clinical team, including medical assistants, behavioral health clinicians, community health workers, and others.
• The successful candidate may be flexible in providing care across our clinic system, working with multiple colleagues from week to week, or may be located in a home clinic.
Qualifications
• Active Licensed PA or NP in the state of Oregon
• Current DEA License
• 2 or more years of experience, preferably in rural community medicine
