APP Post Acute Care opening in Bronx, NYNew York CityFull-time, EmployedWe are seeking a quality-driven Advanced Practice Clinician to join our post-acute care team in the Riverdale neighborhood of The Bronx, New York. This is a full-time role (5 days/week) rounding in skilled nursing facilities, with excellent support and training, autonomy, and earning potential.

Responsibilities:- Perform comprehensive patient assessments, including medical histories and physical exams
- Develop, implement, and adjust individualized care plans based on patient needs
- Monitor progress and coordinate care to optimize outcomes
- Collaborate closely with physicians, nursing staff, and other healthcare professionals in a team approach
- Educate patients and families on health management and preventive strategies
- Maintain accurate, timely medical records in compliance with regulations
Compensation & Benefits- Compensation is fee for service with an estimated compensation range of $132,000 - $156,000 annually and no cap on productivity income potential.

Qualifications- Experience in post-acute, acute, ED, or clinic settings
- Current NP/PA license (State of New York) and DEA
