APP - Family Medicine Opening in Lebanon, OR-Full Time & Permanent Position.Seeking a physician assistant or nurse practitioner to join our Family Medicine team in Lebanon, Oregon. Clinic is an established certified Rural Health Clinic with experienced clinicians who are passionate about building healthier communities within a certified Patient Centered Medical Home model of care.Job Specifics-1.0 FTE (34 hours per week).-Four day work week, no weekends.-Up to 20 patients per day.-Call schedule: phone call only with 1-2 days a month plus one weekend every 2-3 months.-Outpatient Family Medicine - mixture of preventative and acute visits.Job Requirements-Excellent clinical skills, communication skills, and a strong commitment to providing excellent care that is team oriented.-Eligibility to work in the United States.-Graduate of an accredited Physician Assistant program/Nurse Practitioner program.-Licensed in the State of Oregon.-Experience required.The group offers an attractive salary plus incentives, relocation assistance, paid malpractice, CME allowance, and a competitive benefits package.Location detailsOur Park Street Clinics is affiliated with the Samaritan Lebanon Community Hospital, which is a full-service, 25-bed critical access hospital.
