• BLS (Basic Life Support)
• DEA Controlled Substance Registration Certificate
• Board Certification (or Board Eligible)
• Malpractice History Verification (NPDB/FSMB)

• Willing to License: No ? Active Michigan license required
• Estimated Privileging Timeline: TBD
