Requirements:
  • Physicians must be board certified in their specialty, or board eligible and will become board certified within 5 years of becoming board eligible - REQUIRED (no exceptions)
  • Valid MI license - REQUIRED - at time of employment
  • What Fellowship (if required): Diabetes, Endocrinology & Metabolism - REQUIRED
  • DEA - REQUIRED
  • BLS Certification and maintain recertification - REQUIRED
  • Most recent NPDB Self Query Report (within 30 days active) - REQUIRED
  • Board Certified
  • Board Eligible
  • DEA
