We are seeking an Internal Medicine Physician to join a growing healthcare organization providing comprehensive medical care to patients in inpatient rehabilitation and skilled nursing settings.
This is an excellent opportunity for an Internal Medicine physician who enjoys managing medically complex patients, collaborating with an interdisciplinary care team, and building meaningful relationships with patients throughout their recovery.
Position Responsibilities
- Evaluate and manage new patient admissions.
- Provide comprehensive Internal Medicine care to patients in inpatient rehabilitation and skilled nursing settings.
- Develop individualized treatment plans based on each patient's medical needs and rehabilitation goals.
- Manage acute and chronic medical conditions, including complex and multiple comorbidities.
- Provide ongoing medical oversight and collaborate with advanced practice providers responsible for daily patient care.
- Monitor patient progress and adjust treatment plans as clinically appropriate.
- Coordinate care with nursing, therapy, and other members of the interdisciplinary team.
- Promote disease prevention, wellness, and improved clinical outcomes.
- Provide compassionate, patient-centered care to a predominantly geriatric population.
- Maintain accurate and timely clinical documentation.
Qualifications
- Board Certified or Board Eligible in Internal Medicine.
- Active medical license in the applicable state.
- Experience in inpatient rehabilitation, skilled nursing, geriatrics, hospital medicine, or management of medically complex patients is preferred.
- Strong understanding of adult and geriatric medicine.
- Excellent clinical judgment and communication skills.
- Comfortable working collaboratively with advanced practice providers and multidisciplinary healthcare teams.
