Requirements:
  • TRULY Board Eligible (within 5 years of residency) or Board Certified in OB/GYN - REQUIRED
  • Active TX license (or in process) - REQUIRED
  • Clean malpractice and license history - HIGHLY PREFERRED
  • Most recent NPDB Self Query Report (within 30 days active) - REQUIRED no exemption, required at presentation
  • Is the provider interested in full-time or part-time employment? (note in "Additional Submission Details" field) - REQUIRED
  • Does the provider require visa support - J1 or H1B? (note in "Additional Submission Details" field) - REQUIRED
  • DEA (must be obtained prior to start) - (note in "Additional Submission Details" field) REQUIRED no exemptions
  • Is the provider interested in full-time or part-time employment?
  • Does the provider require visa support - J1 or H1B?
  • Board Certified
  • Board Eligible
