Family Practice/Primary Care Nurse Practitioner

Aya Locums is seeking experienced Physician Assistants and Nurse Practitioners for ongoing Home Health locum opportunities across Connecticut.

This is a flexible opportunity offering both short-term and long-term assignments with full-time or part-time scheduling options available.

• Flexible scheduling available Monday Sunday
• No nights or weekend requirements
• Hours: 7:00 AM 7:00 PM
• Average of 6 9 patient visits per day
• Ongoing coverage need
• Opportunities available in and around:
  • Bridgeport
  • Hartford
  • New London

Providers will conduct pre-scheduled in-home health evaluations for Medicare Advantage, Medicaid, and select commercial plan members. Visits include:

• Medication review
• Medical history review
• Physical evaluation completed in the patient s home

• Active Connecticut license
• Certified PA or NP
• Willingness to travel throughout at least 3 counties
