Physician - Primary Care / Post-Acute Care
Location: Western, MA

Schedule: Monday-Friday Round-and-Go Model
Position Overview
We are seeking a dedicated Primary Care Physician (MD/DO) to provide comprehensive care to patients in skilled nursing, assisted living, memory care, and post-acute settings. This role offers a flexible round-and-go schedule, clinical independence, and opportunities for additional income through Medical Directorships.
Key Responsibilities
- Provide primary care to geriatric and post-acute patients.
- Manage acute and chronic medical conditions.
- Conduct examinations, manage medications, and order labs/imaging.
- Coordinate care with nurses, NPs, PAs, and other healthcare professionals.
- Visit assigned facilities approximately twice per week.
- Participate in limited rotation-based call coverage.
- Opportunity to serve as Medical Director for assigned facilities.
Qualifications
- MD or DO with training in Family Medicine, Internal Medicine, or Geriatrics.
- Active or eligible Rhode Island medical license.
- Active DEA license.
- Skilled nursing, post-acute, or geriatric experience preferred.
- New graduates with geriatrics or post-acute exposure may be considered.
- Ability to work independently and collaboratively.
