What You'll Do
- Provide comprehensive primary care, including preventive, acute, and chronic disease management
- Build long-term relationships with patients and families
- Partner with Whole Health Coaches and care team members to develop personalized care plans
- Use evidence-based medicine and population health strategies to improve outcomes
- Empower patients through education, wellness coaching, and shared decision-making
Qualifications
- MD or DO
- Board Certified or Board Eligible in Family Medicine, Internal Medicine, or related primary care specialty
- Active Georgia medical license (or ability to obtain)
- DEA registration (or ability to obtain)
- Passion for relationship-based, patient-centered care
