Facility:This community health center is a federally qualified health center dedicated to providing accessible, high-quality primary care services to a diverse patient population. The facility operates Monday through Friday and serves a mix of pediatric and adult patients in a collaborative and supportive clinical environment.
Opportunity:Advanced Practice - Physician Assistant-Family Practice
Job Details:- Provide comprehensive family practice care to a mixed pediatric and adult patient population
- 18 patient encounters per shift, Monday through Friday, 8:00 AM - 5:00 PM
- Chart closure required within 72 hours
- Must have a minimum of 3 years of experience in a similar clinical setting
- Active California license required; licensed in CA only
- Active DEA required prior to start date
- PAs must have a CA Controlled Substance Education Course Certificate
- NPs must have a Furnishing License with Schedule II
- BLS certification required
- Providers must be ORP (order, referring, and prescribing) enrolled in MediCal or willing to enroll during credentialing
- Candidate CVs must include month and date on all work experience with all gaps in employment accounted for
- Required immunizations prior to start date: Flu Vaccine, TB Status/X-Ray Results (within 90 days of start), Positive MMR Titer or Complete Immunization Series, Positive Hep B Titer or Complete Immunization Series, Positive Varicella Zoster Titer or Complete Immunization Series, and Tdap Immunization (within the last 10 years)
