About the Opportunity
A leading home-based primary care provider in the Greater Boston area is seeking a motivated Family Nurse Practitioner to deliver comprehensive, compassionate care to patients directly in their homes. New Graduate NPs are strongly encouraged to apply! This direct-hire position offers extensive orientation, structured clinical mentorship, manageable daily patient volume, and flexible scheduling that prioritizes work-life balance.
Key Responsibilities
- Conduct comprehensive in-home health assessments, routine physicals, and disease management for adult and geriatric patients.
- Diagnose and manage acute and chronic conditions (e.g., diabetes, hypertension, CHF, COPD) within the home setting.
- Order, interpret, and review diagnostic tests, labs, and medications.
- Collaborate closely with supervising physicians, home health agencies, social workers, and family caregivers to ensure coordinated care.
- Utilize mobile EHR technology for efficient real-time charting and documentation.
Qualifications
- Master s Degree in Nursing (MSN) from an accredited program.
- Active, unencumbered Massachusetts Nurse Practitioner License (or in progress).
- Board Certification as a Family Nurse Practitioner (FNP-BC or AANP) or Adult-Gerontology Primary Care NP (AGPCNP).
- Active Massachusetts DEA and Controlled Substance Registration (CSR).
- Valid Driver s License with a reliable vehicle and clean driving record.
- Passion for community-based, patient-centered care and strong autonomy.
