- An established home-based primary care medical group in Brooklyn, NY, is seeking a qualified House Call Physician Assistant to join their growing clinical team. This full-time, direct-hire position offers an autonomous practice environment with manageable daily patient volume, dedicated administrative support, and flexible scheduling, allowing you to provide comprehensive, patient-centered care to adult and geriatric populations in their homes.
- Key Responsibilities
- Conduct comprehensive in-home medical assessments, routine physicals, and ongoing primary care management for adult and geriatric patients.
- Diagnose and manage acute illnesses and complex chronic conditions (e.g., hypertension, diabetes, CHF, COPD, neurodegenerative disorders) in the home setting.
- Order, interpret, and review laboratory work, diagnostic testing, and mobile imaging studies (X-rays, ultrasounds).
- Formulate evidence-based treatment plans, prescribe medications, and manage home health/durable medical equipment (DME) orders.
- Collaborate closely with supervising physicians, home health agencies, physical therapists, and family caregivers to ensure seamless care coordination.
- Maintain prompt and accurate clinical documentation using mobile electronic health record (EHR) systems.
Qualifications
- Graduation from an accredited Physician Assistant (PA) Program.
- Active, unencumbered New York State Physician Assistant License.
- NCCPA Certification (PA-C).
- Active New York DEA registration and Prescriptive Authority.
- Minimum 1+ years of clinical experience in Primary Care, Family Medicine, Geriatrics, Internal Medicine, or Home Health preferred.
