What You ll Do - Provide compassionate hospice and palliative care to patients in accordance with best practices
- Collaborate with residents and healthcare team members to develop and implement care plans
- Engage in patient and family education regarding disease management and end-of-life options
- Participate in precepting and mentoring residents and healthcare staff, if desired
- Ensure compliance with all healthcare regulations and facility standards
- Document patient interactions accurately and timely in electronic health records
What You Need - Active medical license in Virginia
- Certification or eligibility in Hospice and Palliative Medicine preferred
- Strong communication and interpersonal skills
- Ability to work collaboratively within a multidisciplinary team
- Commitment to providing high-quality, patient-centered care
