Key Responsibilities
  • Provide comprehensive primary care services to adult and senior populations
  • Evaluate, diagnose, and manage acute and chronic medical conditions
  • Coordinate specialty referrals and transitions of care
  • Collaborate with multidisciplinary care teams through daily clinical huddles
  • Participate in quality improvement initiatives and value-based care programs
  • Maintain timely and accurate EMR documentation
  • Support preventive care and population health management efforts
  • Help drive a culture of collaboration, accountability, and clinical excellence
Qualifications
  • MD or DO from an accredited medical school
  • Board Certified or Board Eligible in:
    • Family Medicine
    • Internal Medicine
    • Geriatric Medicine
  • Active, unrestricted medical license (or ability to obtain)
  • Active DEA License
  • Medicare and Medicaid Provider Eligibility
  • Strong communication and relationship-building skills
  • Passion for team-based, value-based patient care
