Key Responsibilities
- Provide comprehensive primary care services to adult and senior populations
- Evaluate, diagnose, and manage acute and chronic medical conditions
- Coordinate specialty referrals and transitions of care

- Participate in quality improvement initiatives and value-based care programs

Qualifications
- MD or DO from an accredited medical school
- Board Certified or Board Eligible in:
- Family Medicine
- Internal Medicine
- Geriatric Medicine
- Active, unrestricted medical license (or ability to obtain)
- Active DEA License
- Medicare and Medicaid Provider Eligibility
- Strong communication and relationship-building skills
- Passion for team-based, value-based patient care
