- Seeking a board certified physiatrist
- Must have an active Illinois medical license
- Must be available to cover the entire requested assignment dates
- Inpatient rehabilitation coverage
- Coverage dates:
- December 19-27
- February 15-21
- 18-bed inpatient rehabilitation unit
- Average daily census of approximately 11 patients
- Common diagnoses include:
- Orthopedic conditions
- Stroke
- General medical/surgical rehabilitation
- EMR: Epic
- ACLS required (or willing to obtain)
Credentialing Requirements:At the time of credentialing, applicants must demonstrate one of the following:
- Provision of inpatient, outpatient, or consultative PM&R services for at least 50 patients within the past 12 months reflective of requested privileges, or
- Successful completion of an ACGME- or AOA-accredited residency, fellowship, or clinical research program within the past 12 months
