high-quality care for Medicare-aged patients through smaller patient panels, longer visit times, and strong clinical support teams.

Minimal call (shared statewide rotation)

Responsibilities• Provide comprehensive primary care services for adult patients
• Diagnose and manage acute and chronic medical conditions
• Conduct preventative care visits and health screenings
• Develop treatment plans and coordinate follow-up care
• Collaborate with interdisciplinary care teams
• Document patient encounters using
Epic EMRRequirements•
MD or DO• Board Certified or Board Eligible in
Family Medicine• Active
Ohio medical license• Active
DEA licensePreferred qualifications:
• Experience in primary care or outpatient medicine
• Experience working with
Medicare or geriatric populations• Familiarity with value-based care models
