Position Summary
We are seeking a dedicated and compassionate Skilled Nursing Facility (SNF) Physician to provide high-quality medical care to residents across skilled nursing facilities in Northern Colorado.

Key Responsibilities
Provide comprehensive medical care to residents in skilled nursing facilities.
Evaluate, diagnose, and manage acute and chronic medical conditions.
Develop and implement individualized treatment plans.
Collaborate with nursing staff, specialists, and interdisciplinary care teams.
Work 3-4 days per week with 10-hour shifts.
Participate in 3-4 on-call shifts per month, including nights and weekends.
Provide medical guidance and support during on-call coverage
Qualifications
Doctor of Osteopathic Medicine (DO) degree from an accredited medical school.
Board Certified or Board Eligible physician.
Active medical license or eligibility to obtain licensure.
Current BLS certification.
Experience in skilled nursing, geriatrics, post-acute care, or internal/family medicine preferred.
