Position Overview

Horizon Family Medical Group is seeking a Nurse Practitioner (NP) to join a thriving nephrology practice in the beautiful Hudson Valley region of New York.

Coverage includes:

  • New Windsor, NY (Outpatient Nephrology Clinic)
  • Newburgh, NY (St. Luke s Cornwall Hospital Montefiore)

Scenic region with easy access to NYC, New Jersey, Connecticut, and Pennsylvania

Clinical Responsibilities
  • Manage chronic kidney disease, hypertension, electrolyte disorders, and renal conditions
  • Provide inpatient nephrology consultations and hospital rounding
  • Conduct daily dialysis rounding (acute & chronic dialysis patients)
  • Order and interpret labs, imaging, and diagnostics
  • Develop individualized treatment plans
  • Educate patients and families on kidney disease management and diet
  • Collaborate with nephrologists, dialysis teams, and multidisciplinary staff
  • Maintain accurate EMR documentation
  • Participate in quality improvement initiatives

Qualifications
  • Current New York State Nurse Practitioner license
  • National certification (ANCC or AANP) required
  • Master s or Doctorate in Nursing required
  • Active DEA license required
  • Minimum 2 years nephrology experience required
  • Experience in both outpatient and inpatient nephrology care
  • Dialysis management experience required
  • Strong clinical reasoning and diagnostic skills
  • Excellent communication and teamwork abilities
Preferred Qualifications
  • Hospital rounding experience
  • Dialysis procedure familiarity
  • EMR experience
  • BLS & ACLS certification
