  • Board Certified Orthopedic Surgery or Truly Board Eligible (within 0-5 years of residency completion) - REQUIRED
  • CO license (must be obtained prior to start) - HIGHLY PREFERRED
  • Clean malpractice - HIGHLY PREFERRED
  • Most recent NPDB Self Query Report (within 30 days active) - REQUIRED no exemption, required at presentation
  • Does the provider require visa support - J1 or H1B? (note in "Additional Submission Details" field) - REQUIRED
  • DEA (must be obtained prior to start) - (note in "Additional Submission Details" field) - REQUIRED no exemptions
