LPN MDS Coordinator
Job Summary
As an LPN MDS Coordinator at Green Palms Health & Rehab of St. Marys, you will play a vital role in ensuring the high-quality care and comfort of our residents. Responsibilities
Coordinate the completion of the Minimum Data Set (MDS) assessments for our residents, ensuring compliance with regulatory requirements and company standards
Collaborate with the healthcare team to develop and implement individualized care plans that address the unique needs and goals of each resident
Monitor and document changes in resident condition, including physical, cognitive, and emotional status
Communicate with residents, families, and healthcare providers to ensure accurate and timely information exchange
Utilize your strong organizational skills to manage and prioritize tasks, ensuring seamless coordination of care
Maintain accurate and up-to-date records, adhering to HIPAA and regulatory guidelines
Participate in quality improvement initiatives, contributing to the growth and development of our care team
Requirements
Licensed Practical Nurse (LPN) license in the state of Ohio
High school diploma or equivalent required; ADN or RN degree preferred
0-2 years of experience in long-term care or similar setting, with a focus on MDS coordination
Strong communication, organizational, and problem-solving skills
Ability to work effectively in a fast-paced environment, prioritizing tasks and managing multiple responsibilities
