Current Opportunity
We are seeking a PRN Registered Nurse, preferably, or Licensed Practical Nurse to provide intermittent skilled nursing visits in the South Bend area, with the ability to assist with visits in Elkhart when needed.
An RN is preferred to assist with patient admissions; however, qualified LPN candidates are also encouraged to apply.
Current Scheduling Needs
Two patient visits every other Friday
One patient visit every other Sunday
Additional PRN visits may become available as patient needs grow
Nursing Responsibilities
Provide intermittent skilled nursing visits in patients' homes
Complete nursing assessments and monitor changes in patient condition
Follow individualized plans of care and physician orders
Provide education and support to patients and their families
Maintain accurate and timely clinical documentation
Communicate with physicians, care coordinators, and other members of the care team
Assist with admissions and start-of-care visits when qualified and needed
Travel to South Bend and occasionally Elkhart for scheduled visits
Qualifications
Active Indiana RN or LPN license in good standing
RN preferred, particularly for admission visits
Previous home health, skilled nursing, or geriatric care experience preferred
Strong assessment, communication, and documentation skills
Dependable, compassionate, and professional
Reliable transportation and a valid driver's license
Ability to pass all required background screenings
