About the Role:
Rehab at Scottsdale Village Square is seeking a dedicated and compassionate Evening Shift Licensed Practical Nurse (LPN) to join our healthcare team. Responsibilities:
Provide direct patient care, including administering medications, wound care, and treatments as prescribed by physicians
Develop and implement individualized care plans, collaborating with interdisciplinary teams and healthcare professionals
Monitor and document patient progress, reporting any changes or concerns to medical staff
Maintain accurate and detailed records, adhering to HIPAA guidelines and facility policies
Communicate effectively with patients, families, and healthcare team members to ensure seamless care transitions
Participate in quality improvement initiatives, contributing to evidence-based practice and positive patient outcomes
Requirements:
Currently licensed as a Licensed Practical Nurse (LPN) in the state of Arizona
High school diploma or equivalent required; Associates degree or higher in Nursing preferred
Minimum 1 year of experience in long-term care or post-acute care settings
Ability to work effectively in a fast-paced environment, prioritizing patient needs and tasks
Excellent communication, interpersonal, and organizational skills
Ability to maintain patient confidentiality and adhere to HIPAA guidelines
