Responsibilities
As an LPN, you'll be an integral part of our clinical team, delivering high-quality care and helping our residents achieve their highest level of wellness and independence.
Your Responsibilities Will Include:
Administering medications and treatments safely and accurately
Coordinating resident admissions, discharges, and transfers
Communicating with physicians, pharmacies, residents, and families
Monitoring residents' conditions and reporting changes promptly
Collaborating with the interdisciplinary care team to provide exceptional resident outcomes
Implementing and evaluating individualized care plans
Maintaining accurate and timely clinical documentation
Following infection control, safety, and emergency procedures
Supervising nursing assistants and support staff as appropriate
Ensuring resident dignity, privacy, and rights are always protected
Maintaining knowledge of nursing equipment and proper usage
Providing compassionate nursing care in a long-term care and rehabilitation setting
Qualifications
Current Licensed Practical Nurse (LPN) License in the State of New Jersey
CPR Certification Required
Strong communication and teamwork skills
Compassionate, dependable, and resident-focused
