About the Role:
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our night shift team. As an LPN with Green Palms Health & Rehab at Crawford, you will play a vital role in delivering high-quality care to our residents. Your responsibilities will include:
Assisting with the delivery of patient care, including medication administration, wound care, and vital sign monitoring
Providing emotional support and social interaction to our residents
Collaborating with other healthcare professionals to develop and implement individualized care plans
Maintaining accurate and detailed records of patient care and progress
Responsibilities:
Provide direct patient care, including administering medications, treatments, and procedures as ordered by physicians
Conduct vital sign checks and report any changes or concerns to the nursing staff
Assist with activities of daily living (ADLs) such as bathing, dressing, and grooming
Maintain a clean and safe environment for our residents and staff
Participate in resident care conferences and family meetings
Follow established policies and procedures for infection control, fire safety, and emergency procedures
