A leading Federally Qualified Health Center just southwest of Rutland, Vermont is seeking an experienced Nurse Practitioner or Physician Assistant to provide high-quality primary care across multiple community-based clinic locations while enjoying a collaborative team environment and an exceptional work-life balance.
Position Highlights:
- Provide comprehensive primary care across multiple clinic locations
- Manage acute, chronic, and preventive care for patients of all ages
- Perform comprehensive exams, diagnoses, and treatment planning
- Order and interpret labs and diagnostic testing
- Perform office procedures, including suturing and wound care
- Prescribe medications and provide patient education
- Coordinate care with physicians and multidisciplinary teams
- Utilize integrated behavioral health and care management resources
- Practice within a Level III Patient-Centered Medical Home model
- Fully integrated EMR with strong clinical support
- 0.9 FTE with a 4-day work week
- Limited call averaging just 3 weeks per year
Qualifications:
- Nurse Practitioner or Physician Assistant
- Graduate of an accredited NP or PA program
- Active Vermont licensure or ability to obtain
- 3 5 years of primary care experience preferred
- Strong clinical assessment and diagnostic skills
- Excellent communication and teamwork abilities
- Commitment to high-quality, patient-centered care
Compensation & Benefits:
- Competitive salary: $125K $165K
