A leading community-based healthcare organization in Vermont is seeking an experienced Psychiatric Nurse Practitioner to provide outpatient psychiatric care for adolescents within a collaborative, integrated behavioral health program.
Position Highlights:
- Provide outpatient psychiatric care for adolescent patients
- Diagnose, treat, and manage a variety of behavioral health conditions
- Develop individualized treatment plans focused on patient goals
- Collaborate with therapists, case managers, physicians, and support staff
- Perform psychiatric evaluations, medication management, and follow-up care
- Review diagnostic information and monitor treatment progress
- Provide patient and family education on medications, wellness, and mental health strategies
- Coordinate referrals and continuity of care
- Utilize an integrated electronic medical record system

Qualifications:
- Board Certified Psychiatric Mental Health Nurse Practitioner (PMHNP-BC)
- Active state licensure as an Advanced Practice Registered Nurse
- 1 2 years of psychiatric or behavioral health experience preferred
- Strong assessment, diagnostic, and clinical decision-making skills
- Excellent communication and interpersonal abilities
- Passion for working with adolescents and families
- Commitment to collaborative, community-based care
