As a valued member of the Berks Community Health Center (BCHC) team, the Certified Registered Nurse Practitioner (CRNP) provides comprehensive, patient-centered primary and preventive care for individuals and families across the lifespan in collaboration with a physician. The CRNP leads and coordinates care teams to manage acute illnesses, chronic conditions, and wellness needs while promoting high-quality patient outcomes. This role collaborates with interdisciplinary teams, participates in quality initiatives, and supports BCHC s mission as a Federally Qualified Health Center (FQHC) and Patient-Centered Medical Home (PCMH), delivering exceptional care to all residents of Berks County regardless of ability to pay.
Principal duties & Responsibilities:
- Provide comprehensive primary care, including preventive, acute, and chronic disease management.
- Evaluate patients through assessments, medical histories, diagnostics, and treatment planning.
- Prescribe and manage medications and treatment plans within scope of practice.
Maintain accurate, timely documentation in the electronic health record (EHR). - Lead and collaborate with interdisciplinary care teams to coordinate high-quality patient care.
- Support care coordination, patient education, and self-management strategies.
Identify and manage high-risk patients in collaboration with physicians and care team members. - Lead daily team huddles and support efficient workflows and patient flow.
Participate in quality improvement, patient safety, peer review, and evidence-based practice initiatives. - Foster strong patient relationships while supporting BCHC s mission, values, and patient-centered care model.
Education - Master of Science, Nursing Certified Family Health Practitioner
License & Certifications
- Pennsylvania State Board of Nursing Registered Nurse
- Certified Registered Nurse Practitioner
- Prescriptive Authority
- BCLS
Training and Experience
- Minimum one year nurse practitioner experience preferred.
