Position Summary

The PA Primary Care Career Center was founded to match up primary care providers (physicians, nurse practitioners, physician assistants, dentists, and more) with organizations that provide primary care services. As a valued member of the Berks Community Health Center (BCHC) team, the Certified Registered Nurse Practitioner (CRNP) provides comprehensive, patient-centered primary and preventive care for individuals and families across the lifespan in collaboration with a physician. The CRNP leads and coordinates care teams to manage acute illnesses, chronic conditions, and wellness needs while promoting high-quality patient outcomes. This role collaborates with interdisciplinary teams, participates in quality initiatives, and supports BCHC s mission as a Federally Qualified Health Center (FQHC) and Patient-Centered Medical Home (PCMH), delivering exceptional care to all residents of Berks County regardless of ability to pay.

Principal duties & Responsibilities:

  • Provide comprehensive primary care, including preventive, acute, and chronic disease management.
  • Evaluate patients through assessments, medical histories, diagnostics, and treatment planning.
  • Prescribe and manage medications and treatment plans within scope of practice.
    Maintain accurate, timely documentation in the electronic health record (EHR).
  • Lead and collaborate with interdisciplinary care teams to coordinate high-quality patient care.
  • Support care coordination, patient education, and self-management strategies.
    Identify and manage high-risk patients in collaboration with physicians and care team members.
  • Lead daily team huddles and support efficient workflows and patient flow.
    Participate in quality improvement, patient safety, peer review, and evidence-based practice initiatives.
  • Foster strong patient relationships while supporting BCHC s mission, values, and patient-centered care model.
Education
  • Master of Science, Nursing Certified Family Health Practitioner
License & Certifications
  • Pennsylvania State Board of Nursing Registered Nurse
  • Certified Registered Nurse Practitioner
  • Prescriptive Authority
  • BCLS
Training and Experience
  • Minimum one year nurse practitioner experience preferred.
