- Requirements: 2+ years of experience as a PTA; at least 2 years of experience in Home Health; Valid Physical Therapy Assistant license; Be willing to obtain STATE licensure
Responsibilities:
- Deliver one-on-one, high-touch care to patients in their homes
- Conduct skilled nursing visits and perform assessments
- Create and manage individualized care plans
- Collaborate with families and interdisciplinary providers to improve patient outcomes
