Practice Model: Outpatient only, value-based care, no hospital responsibilities

Ideal Candidate
- Board Certified/Board Eligible in Family Medicine or Internal Medicine
- Experience caring for Medicare Advantage patients
- Comfortable managing complex chronic conditions such as diabetes, CHF, COPD, obesity, and cardiovascular disease
- Familiarity with ICD-10 coding and STAR quality measures
- Experience with eClinicalWorks EMR is a plus
- Adept at working Independently
- Must be Bilingual English/Spanish
