Overview:
Our outpatient care team is seeking a Physician Assistant to provide comprehensive patient care focused on chronic disease management and treatment planning. Our role offers the opportunity to collaborate with healthcare professionals while developing diagnostic interpretation skills through on-the-job training. New graduates are encouraged to apply.
Position Offers:
- Schedule: Monday Friday, 8:00 AM 5:00 PM, full-time
- Annual salary: $110,000 $120,000

Key Responsibilities:
- Conduct comprehensive patient assessments and clinical evaluations in an outpatient setting
- Develop, implement, and manage treatment plans for patients with chronic conditions
- Monitor patient progress and adjust medications as appropriate
- Coordinate ongoing care and follow-up services to support disease management
- Interpret diagnostic testing with training provided as needed
- Collaborate with physicians and interdisciplinary team members to optimize patient outcomes
- Educate patients on treatment plans, medications, and disease management strategies
- Maintain accurate, timely, and compliant clinical documentation
Qualifications:
- Physician Assistant licensure
- Experience in primary care, chronic disease management, or specialty care preferred
- Strong clinical assessment and patient management skills
- Ability to work effectively in a fast-paced outpatient environment
- Commitment to providing high-quality, patient-centered care
