Requirements
- MD or DO degree from anaccredited medical school
- Board Certified or BoardEligible in Family Medicine
- Completion of an accreditedFamily Medicine residency program
- Eligible for or currentlyholding an unrestricted Indiana medical license
- DEA certification oreligibility to obtain
- Excellent communication,interpersonal, and clinical skills
