Please ask about our generous referral fees!Seeking an experienced Family Nurse Practitioner (FNP) or Physician Assistant (PA)100% outpatient primary care practiceMonday - Friday, 8:00 AM - 5:00 PMMinimal call responsibilitiesPatient population is exclusively adults ages 65+Average daily patient volume: 12-15Value-based care model with a strong emphasis on quality outcomes and patient-centered careCollaborative care team includes a full-time physician, nurse practitioners, medical assistants, and care coachesCandidates who are fluent in Spanish are strongly preferred, as the practice serves a large Spanish-speaking patient population. However, candidates with strong value-based care (VBC) experience will be considered in lieu of being Spanish-speaking.
