Responsibilities
Conduct pre-scheduled in-home health evaluations for Medicare Advantage, Medicaid, and select commercial health plan members.
Visits include:
- Comprehensive medical and medication history review
- Physical health assessment
- Completion of patient-specific documentation provided prior to each visit
This is a non-treatment role:
- No prescribing medications
- No ordering labs or diagnostic testing
- No modifying existing treatment plans
Schedule & Volume
- Flexible scheduling options
- Average of 6 9 patient visits per day
Requirements
Active NP or PA license in the state of practice
Board Certification
Medicare/Medicaid eligible
Credentialing
- Approximately 30-day credentialing process
