Nurse Practitioner (NP) / Physician Assistant (PA) - Primary Care

Location: Elk Grove, CA
Job Type: Part-Time (20-25 Hours/Week)
Setting: Outpatient Primary Care / Family Medicine
Patient Population: Adults, Seniors, Medicare, Medi-Cal, and Commercial Insurance

About the Opportunity:

We are seeking a compassionate Nurse Practitioner (NP) or Physician Assistant (PA) to join a growing outpatient primary care practice in Elk Grove, CA. This role offers the opportunity to provide high-quality, patient-centered care in a collaborative environment focused on preventive medicine, chronic disease management, and continuity of care.

Key Responsibilities

  • Provide comprehensive outpatient primary care services.

  • Evaluate, diagnose, and treat acute and chronic medical conditions.

  • Perform physical examinations and patient assessments.

  • Order and interpret laboratory tests and diagnostic studies.

  • Prescribe medications in accordance with California regulations.

  • Conduct preventive care visits, annual wellness exams, and chronic disease management.

  • Educate patients on treatment plans and healthy lifestyle choices.

  • Coordinate specialty referrals and follow-up care.

  • Maintain accurate and timely EMR documentation.

  • Collaborate with physicians and the clinical team to deliver quality patient care.

Qualifications

Required:

  • Active California Nurse Practitioner or Physician Assistant license.

  • Strong clinical assessment and communication skills.

  • Comfortable using Electronic Medical Records (EMR).

Preferred:

  • Experience in Family Medicine, Primary Care, Internal Medicine, Adult Medicine, or Urgent Care.

  • DEA registration and BLS certification.

  • Experience with Medicare Wellness Visits, Preventive Care, Chronic Disease Management, and HEDIS/IHA quality measures.

New Graduates: Strong new graduates with primary care or outpatient clinical training are encouraged to apply.
